Learning Outcomes Assessment

The links found below will take you to forms to be used for collecting data associated with the Learning Outcomes Assessment process in the University of Maryland School of Music. For most of the document you will need to first download the appropriate form (by clicking on the link) prior to the related event. Then complete and return it to the School of Music Student Services Office no later than two weeks following the event.


Undergraduate: to be completed by the student’s applied teacher after the required recital, hearing, or jury in the student’s final semester of lessons. (BA: MUSP305; BM: MUSP420; BME: MUSP410)

Graduate: to be completed by the student’s applied teacher in the semester in which the student gives his or her final degree recital or hearing.

To be completed by the student’s primary advisor immediately following the student’s defense of a thesis or dissertation.

BM Composition Form: to be completed by the students composition lesson instructor during the student’s final semester of lessons (MUSP420).

Undergraduate LOA: BM Composition Form

Graduate Preliminary Exam Form: to be complete by faculty readers as they evaluate doctoral preliminary exams.

Graduate Scholarly Paper Form: to be completed by the instructor of MUSC648 and MUED690 on behalf of all students enrolled in a Master of Music degree program.
